How to correctly complete the monthly contributions statement?

  • In the regular monthly statement, include all employees:
    • with a legal relationship with a right to a regular salary;
    • contract workers with a regular salary (including those who have claimed a levy deduction);
    • seasonal contract workers (with both a right to a regular salary and a right to an irregular salary);
       
      – including those with suspended compulsory insurance or exempted from the obligation to pay contributions (e.g. WI, parental leave).
  • Also specify the assessment basis according to income in the statement.
  • Provide details of the contributions paid by the employee in the given period.
  • Provide details of the contributions that you pay as an employer in the given period.
  • Please indicate the number of calendar days.
  • In the event of suspension or exemption from payment of contributions during the whole month, please indicate 0 as the number of calendar days.
  • Check all data and make sure it is correct.
